Calf Circumference

Calf circumference is the maximum distance around the calf, measured while seated with the tape perpendicular to the right lower leg. It is a practical body-composition marker, but the measurement includes skeletal muscle, subcutaneous fat, and intramuscular fat; a larger value does not always mean more muscle.

These percentiles represent US adults in NHANES 1999-2006 and report raw measured circumference. FitnessNorms reproduced the comparison paper's 17,856-person eligible cohort, then applied a conventional global 3 × IQR outlier rule for a final n=17,839 because the paper did not specify how its reported 67 outliers were identified. Population context matters. A multinational study in Japan, Malaysia, and Taiwan found different age-specific calf distributions and a clearer decline after age 60 (Chen et al. 2025). Those values are useful context, not a direct conversion, because cohorts and measurement protocols differ.

  1. Sit with the right lower leg accessible and the foot supported in the examination position.
  2. Locate the largest girth of the right calf.
  3. Place the tape around that point in a plane perpendicular to the long axis of the lower leg.
  4. Keep the tape snug against the skin without compressing the tissue.
  5. Record the maximum circumference to the nearest 0.1 cm.
